<p><strong>Clinical Manager – Assistive Care State Programs | Bloomfield, NJ</strong></p> <p><strong>Salary:</strong> $48.00 per hour</p> <p><strong>Schedule:</strong> Part-time, non-exempt | 30 hours per week | Specific days and daily hours to be confirmed</p> <p><strong>Benefits:</strong> BAYADA benefits package; eligibility and coverage details may vary based on part-time status</p> <p>BAYADA HOME HEALTH CARE is seeking a Registered Nurse (RN) Clinical Manager for our Essex Assistive Care State Programs (ESS) office in Bloomfield, NJ. In this part-time RN leadership role, you will provide clinical oversight for clients receiving state-funded personal care services while supporting and developing a team of Home Health Aides and Personal Care Assistants.</p> <p>Our Assistive Care State Programs services help adults and seniors receive personal care and support at home through Medicaid waiver and other state-funded programs. These services promote safety, independence, and quality of life for clients who may otherwise require a higher level of care or institutional support.</p> <p>If you are an organized, systems-minded RN who is comfortable with program requirements, documentation, and client advocacy, this could be the next step in your career. This Clinical Manager position is ideal for an RN who wants to combine clinical leadership, regulatory compliance, care coordination, and meaningful community impact.</p> <p><strong>What you’ll do</strong></p> <ul> <li>Provide clinical oversight for clients enrolled in state-funded Medicaid waiver and personal care programs</li> <li>Conduct client assessments and develop individualized care plans aligned with client needs and state program guidelines</li> <li>Supervise, support, and provide clinical guidance to Home Health Aides and Personal Care Assistants</li> <li>Ensure care delivery aligns with approved services, authorized care hours, state program requirements, and BAYADA standards</li> <li>Coordinate with state case managers, Medicaid managed care organizations, families, caregivers, and other external stakeholders</li> <li>Support client reauthorization processes and help ensure care hours remain aligned with approved program parameters</li> <li>Review documentation for accuracy, completeness, timeliness, and compliance with applicable state and program requirements</li> <li>Facilitate caregiver training, orientation, and competency documentation as required</li> <li>Monitor quality of care and respond to incidents, complaints, changes in client condition, and service concerns</li> <li>Conduct supervisory visits and follow up on client and caregiver needs</li> <li>Serve as a clinical resource, client advocate, and partner to the Director and Client Services Manager</li> <li>Participate in quality assurance, performance improvement, and audit-readiness activities</li> <li>Build positive relationships with clients, families, caregivers, state partners, and community resources</li> <li>Model The BAYADA Way and help build a positive, mission-driven, team-oriented culture</li> </ul> <p><strong>What you bring</strong></p> <p><strong>Required</strong></p> <ul> <li>Current Registered Nurse (RN) license in good standing in New Jersey</li> <li>Minimum of 2 years of RN experience</li> <li>Strong organization, documentation, communication, and problem-solving skills</li> <li>Ability to manage state program requirements, client needs, and competing priorities</li> <li>Ability to provide clinical leadership and support to caregivers in a community-based s
